GM case 4.                   
A 31 years old male came to OPD with chief compliant of stomach pain.

Case scenario

Chief compliant
A male patient complaint of stomach pain in left side and vomiting from 5 days.

History of present illness.
Patient was apparently asymptomatic 5 days back and he developed stomach pain on left side which is of twisting type and vomiting since 5 days which is watery type 2 times per day.which is non projectile,non bilious and contain food particles the colour of vomiting is white.

History of past illness.
He has no similar history of stomach pain in past.
No history of cough ,chest pain, palpitations ,sweating ,shortness of breath,
Fever,burning mixturation.
Hypertensive from 2 years.

Negative history
Diabetes.
Asthama
Tb
Epilepsy
Thyroid

Family history
His father was hypertensive

Personal history
Diet mixed
Apatite decreased
Sleep decreased
Bowel and bladder normal
Burning mixturation
Alcohol addiction
No allergies

General examination
Conscious ,cohorent, cooperative 
Well built and moderately nourished
Pallor present
Icterus no
Cynosis no
Clubbing no
Lymphedenopathy no
Edema no

Vitals
Tem afebrile
Pulse rate 66bpm
Respiratory rate 15 cycles /min
Bp 140/90

Systemic examination

Inspection
Shape is scaphoid
Flanks free
Umbilicous inverted 
Skin scar are absent
Dilated veins no

Palpation
Inspectory findings are confirmed superficial
No local rise in temperature
Deep
Liver  and spleen 
non tender and non palpable.

Percussion
No fluid thrills
Liver span 11 cm
No dull sound heard

Auscultation
Bowel sound 10 min
Clavicular supra and infra
Mamillary 
Axillary and infra axillary
Suprascapular and infrascapular
All resonate sounds heared

Definitive diagnosis
Gastritis
