Case scenario
My self k.priyanka of 3rd year bds.This is an online eblog to discuss our patient health data taken from his consent .Here we discuss our individual patient problem through series of inputs from available global online community centre with an aim to solve patient problem.

Chief compliant
Male patient of 53 years compliant of discomfort in stomach from 13 days.

History of present illness
Patient was apparently asymptomatic 13 days back and developed shortness of breath during working from 1 month .which is relieving on medication.full stomach discomfort no pain but become tight.insidious and relieve itself after 30 min.Gradual in onset.Distension of abdomen.

History of past illness
No history of similar complaints
No diabetic
No asthama
No epilepsy
No hypertensive

No family history

Diet mixed
Alcoholic 15 yrs,daily
Bidi 15 yrs daily 10
Sleep normal
Apatite normal
Bowel and bladder normal
No allergies

General examination
Pallor no
Icterus no
Clubbing no
Cyanosis no
Lymphadenopathy no
Edema no
 
Systemic examination
Inspection
Caries to all teeth
No flanges
Umbilical normal
No scars
No visible pulses
No distended veins

Palpation
No tenderness
Done in supine position with both limbs flexed hands by side of body.
Abdomen hard
Lower border of liver not palpable
Spleen not palpable
Kidney no
Fluid thrill no
No raised temp

Percussion
No dull sounds heard

Auscultation
Bowel sounds heard.

CHIEF COMPLENT She is suffering from fever and vomiting from 3 days. HISTORY OF PRESENT ILLNESS She has high body temperature.when ever she eats ,she is feeling like nausea and vomiting sensation. HISTORY OF PAST ILLNESSES Dialysis from 3 months.
